Care Navigator

We have an exciting opportunity for a Care Navigator to join our surgery team. We are looking for someone who is enthusiastic about providing high quality care. This role will be predominantly patient facing providing the first point of contact for all patients and acting as a focal point of communication between patients, doctors and other medical staff.

About Us

Milborne Port Surgery is a purpose built, modern GP practice in a pretty Somerset village. Our smaller branch surgery is in the village of Templecombe.

We are a very friendly team with an excellent record of high patient satisfaction. We arrange regular staff social events and whole-practice meetings. We have a wide range of staff working at the practice including GPs, nurse practitioners, practice nurses, health coaches, in-house musculoskeletal physiotherapists, a mental health support worker, midwife and clinical pharmacists.

About the Job

We are looking for a maximum of 30hours a week. Although working patterns will be fixed a degree of flexibility is essential to provide cover for holidays and sickness. Hours of work are between the hours of 08:00 and 18:30 but we also provide one late surgery a week until 20.00.

The successful applicant(s) will work at Milborne Port Surgery and also at our branch site in Templecombe.

A Care Navigator needs to have excellent communication skills, a high level of IT literacy and the ability to relate well to patients, signposting appropriately to colleagues and the wider clinical team. Previous experience in a medical or healthcare environment is helpful but not essential and training will be provided.

Working in a GP practice requires a high level of professionalism and an excellent grasp of the principles of confidentiality and respect for all patients.

Main duties include:

  • Greeting and directing patients arriving at the practice
  • Answering phone calls from patients and other medical professionals
  • Responding to requests using the Askmygp IT system
  • Booking appointments
  • Registering new patients
  • Handling post, paperwork and emails coming into the practice
  • Liaising with our practice dispensary team about patients' prescriptions
  • Dealing with online patient consultation requests and directing these to the appropriate member of the team
  • Copying, scanning and filing of documents and post
  • Assisting with a variety of other administrative tasks to keep the practice running smoothly.

With time there may be the opportunity to develop additional skills and take on new responsibilities as the practice evolves and grows.

Experience:

Essential

  • Experience of working within a team
  • Experience in dealing with members of the public

Desirable

  • Experience in a healthcare or social care setting
  • Experience in a customer-focussed environment

Knowledge and Skills - Essential

  • Excellent verbal communication skills
  • Excellent organisational skills and attention to detail
  • Ability to deal with people who are anxious, upset or unwell
  • Good keyboard skills and familiarity with common IT programmes (e.g. Microsoft Office)
  • Ability to work under pressure and handle several tasks simultaneously
  • Ability to stay calm in an emergency

Disclosure and Barring Service (DBS) Check

This post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975 so all new members of staff must undertake a DBS check. This will be arranged by the practice on successful appointment.

Practice policy is that members of staff cannot be registered as patients so any successful candidate who is a patient of the practice will be required to register with another local GP surgery.

Salary: £11.05 per hour
